RN Care Manager - Remote Hybrid - Long Island | WFH

Job Overview We are seeking a compassionate and driven RN Care Manager to become an integral part of our healthcare team, dedicated to delivering high-quality care coordination for our health plan members in Long Island. This hybrid remote role allows for the flexibility of both home-based work and essential in-home visits, providing you with a unique opportunity to make a profound impact in the lives of our patients. Key Responsibilities • Conduct thorough assessments to evaluate patient needs. • Develop tailored care plans that align with individual health goals. • Coordinate effectively with physicians, hospitals, and various healthcare providers. • Educate and guide patients and their families throughout their healthcare journey. • Advocate for patients to ensure they receive optimal care. • Maintain precise and updated patient records. Required Skills • Current Registered Nurse (RN) license. • Minimum of 2 years’ experience in case management, including managed care or home care. • Strong communication and interpersonal abilities. • Capacity to work independently as well as collaboratively within a team. • Excellent organizational and time management skills. • Bilingual skills (Spanish, Russian, or Chinese) are advantageous. Qualifications • Valid and active Registered Nurse (RN) license. • At least 2 years of relevant experience in case management or a related field.
