TITLE: Quality Improvement Specialist - RN JOB OVERVIEW: Responsible for supporting and facilitating ongoing organization-wide and medical staff quality management and performance improvement programs and activities. DEPARTMENT: Outcomes Management WORK HOURS: Typically, Monday - Friday; Days REPORTSTO: Director, Quality & Patient Safety PREREQUISITES: • Bachelor's degree in nursing or other health care related field of study required; Master's degree preferred. • Registered Nurse, preferred. • Minimum three years' recent experience in acute health care setting, required. • Experience in use of electronic health record (EHR) • Experience in quality management and performance improvement role preferred. • CPHQ certification preferred. QUALIFICATIONS: • Solid understanding of systems thinking, process management and performance improvement. • Organizational and problem-solving ability and skills. • Knowledge of group process, leadership skills, and ability to facilitate performance improvement teams. • Proficiency in application of performance improvement tools and methodologies including aptitude for using QI data management software. • Knowledge of regulatory standards and interpretation including CMS, DOH and TJC. • Ability to prepare effective oral and written reports and presentations to various groups including physicians, hospital leaders and staff. • Proficiency in use of Windows® and MS Office Suites® applications particularly Word and Excel. • Aptitude for navigating quality-related Internet sites and those of various data management vendors and support agencies. Essential Responsibilities and Competencies • Support collection, analyses and interpretation of data for ongoing quality management activities of the organization and medical staff, as assigned. • Collect, analyze and interpret data and information to support decision-making toward performance improvement. • Abstract data from electronic health records (EHR) for purpose of meeting core program requirements for monitoring (eg. quality measures). • Produce accurate, complete and timely data and reports as scheduled or requested. • Function as a resource to physicians and to managers in the analysis, interpretation and use of data for decision-making and performance improvement. • Applying the FOCUS-PDCA model, facilitate process improvement teams designed to improve the quality and safety of patient care. • Demonstrate understanding of team leader roles, facilitator, and other PI team participants. • Demonstrate knowledge and application of group dynamics and leadership strategies. • Apply current literature, relevant research, and knowledge of best practices to develop improvement strategies. • Utilize internal and external comparative data sources to identify improvement opportunities. • Navigate multiple Internet-based sites including, but not limited to AHRQ, TJC, and TJC's Quality Check. • Support the Medical Staff through preparation of mandated ongoing performance measures data and peer review referrals. • Identify and report variations in clinical care that require immediate action. • Apply knowledge of research principles and methods to analyses of data. • Maintain confidentiality of individual patient and physician information. • Support the ongoing development and review of clinical practice policies, procedures, protocols, order sets and forms in accordance with regulatory standards requirements. • Participate actively on assigned standing quality and safety committees of the medical staff, patient care services and the organization. • Provide "just in time" education to physicians, organization leaders and staff regarding quality management and performance improvement strategies and techniques. • Guide the work of clerical support personnel which support QI functions/responsibilities. • Provide suggestions regarding quality improvement opportunities to VP. • Demonstrate awareness of the importance of cost containment for the department. • Network with peers at other organizations within the local community. • Perform other duties as assigned.
