Job Responsibilities:
As a licensed social worker in this remote case management role, you will:
- Collaborate with registered nurses to assess patients' needs and develop personalized care plans
- Research and identify available resources to address patients' financial, transportation, safety, and housing needs
- Empower patients to make informed lifestyle and medical choices through education and advocacy
- Work closely with other clinical staff to provide comprehensive support and resources to patients
- Utilize telephonic and electronic communication to engage with patients and stakeholders
- Maintain accurate and up-to-date records of patient interactions and progress
Requirements:
* Active and unrestricted LSW (Licensed Social Worker), LMSW (Licensed Masters Social Worker), LCSW (Licensed Clinical Social Worker), or LISW (Licensed Independent Social Worker) license in the state of residence
- 2+ years of experience in medical social work or case management
- Master's degree in social work or a related field
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Proficiency in electronic medical records and other relevant software systems
